尊敬的華聯會員們：
大家好，轉眼之間秋意漸濃，2022已近尾聲。華聯會員是採年費制，因此，自即日起，我們開始接受2023年新舊會員會費，仍維持一人一年15元。為方便大家填繳申請表及支票，除了隨函附上申請表格外，我們在各社團也設有志工可以索取及繳交會員表，接受入會繳費。會員亦可將填好的會員表連同支票郵寄本會。新加入的會員可享受2022年餘下的會員活動。

填寫表格時請注意以下事項:
1》村內居民請填寫Manor #及Unit #，村外居民請填寫門牌號碼及Unit #。(秘書組的database是根據以上的號碼，重要，必須填）
2》姓名地址電話號碼及電郵，均請填寫清晰可辨識。（尤其電郵,是聯系各位的基本方式）
3》夫婦或兩人住同一地址可以只填寫一張會員表，但會費是每人15元，兩人30元。為方便帳目登錄，僅收支票，謝謝配合。支票上煩請寫上會員姓名電話。
4》两個（囗囗），這一欄必須選擇一個打勾
。明確表示要不要將資訊登在會員錄上。
備註：不能部份選擇（如只登姓名其他不登⋯。）
5》表格下半頁是免責聲明，請填上姓名，下面會員請簽名及日期。
6》最後一項喜好專長，儘量請填上，為華聯儲備人才。

It’s time of the year that CAC starts accepting membership renewal for 2023. New membership will also be accepted at this time, and new members will be able to join CAC activities for the reminder of 2022 plus entire year of 2023.
Please support CAC and join / renew your membership now! For your convenience, the application form is accessible from our website by clicking the link here: https://lagunawoodsvillage.com…. As well, volunteers will be distributing and accepting membership forms / checks at 14 locations where various clubs meet periodically. Please refer to the list above for the names and contacts of these volunteers.
While filling out the form, please note the following:
1. House / Manor #, Street #, & Unit # are keys to the database records, these are required fields.
2. Please write your name, address, phone #, & e-mail address clearly, especially the e-mail address, because you will receive announcements from CAC primarily via e-mails.
3. Two people at one residence can use one form for both, but the membership fee is $15 per person, $30 for 2 people. Please pay via check only, and write your name and phone # on the check.
4. Please select one of the options to be included or excluded from our member directory. Please note that you cannot select what information to be included; either all or nothing.
5. Please make sure to sign and date the form.
6. Please consider volunteering for CAC and tell us about your skills and hobbies, so we can contact you to join CAC teams.
